A valid prescription order is needed on file before supplies can be shipped. If you have seen a sleep physician within the past year, you most likely have a valid prescription order. To upload your prescription to our system click here

If there is not a valid prescription on file, you will need to obtain a valid order.

If you have seen your primary care physician within the last year, we can send a prescription order to your physician to sign for supplies. Once your physician has signed and returned the order, we can send out your supply order. To have a prescription order sent to your physician, please email with the following information: 

Your First & Last Name, 
Date of Birth, 
Phone Number,
and your Primary Physician’s name with fax number. 

Please also include any updated insurance information.
